Senior Case Manager
Reports To: Director of Operations
ESSENTIAL JOB FUNCTIONS/ DUTIES
The essential functions/duties include, but are not limited to the following:
• Responsible for initial Data Entry in Epp and creating electronic case file for each child.
• Address RFS mailbox to ensure timely notification of case assignment to providers.
• Tracking and daily reporting of First Attend Dates on the State Broadcast System and documentation of any reasons for delays on SBS and EPP.
• Manage following reports: New Student by Month and Active Student Roster to ensures EIP services are being provided at mandated frequency as per authorized IFSP.
• Secure consent forms and any other required documentation for the timely initiation of services.
• Work closely with Program Manager(s) and Clinical team to ensure all progress summaries are completed timely.
• Responsible for addressing Utilization report for designated counties with the goal to achieve 85% or higher utilization.
• Maintains consistent communication with practitioners via phone and / or email and timely response to inquiries.
• Works closely with the billing department to ensure the practitioners are submitting their billing/ SVELOGS each cycle.
• Maintains client files in compliance with contractual requirements; tracks required dates as per all mandated regulations/ policies by the NJ DOH EIP.
• Manage the EI Billing and Notification Log report.
• Documents all follow-ups and communication with practitioners, program managers, and Service Coordinators in Eprovider plus (EPP).
• Provides ongoing communication with service coordinators.
• Attends required meetings and participates in projects or committees as requested.
• Maintains practitioner, client, and company confidentiality.
• Responsible for special projects and additional responsibilities as needed.
• Assist team and program managers with coverage of broadcast as needed.

REQUIREMENTS
• Experience with Early Intervention case management
• At minimum bachelor’s degree, high diploma accepted if relevant EI experience is applicable
PHYSICAL REQUIREMENTS
• Hand dexterity ability (ability to operate mobile device, telephone, computer)
• Ability to sit for extended periods of time
